If a dementia diagnosis is necessary and has been confirmed, I will meet with you and your family to provide guidance. In contrast to other medical diagnoses, a dementia diagnosis frequently leaves family members and caregivers confused and concerned about the next steps for their loved one.
Education: I will provide education on the dementia diagnosis for the caregiver and family member(s). If there are concerns about the diagnosis I can refer for additional follow up assessment to confirm the diagnosis.
Plan of action: I will provide input into important next steps that include: Living arrangements for your loved one (assisted living, nursing home, in-home care), referrals for will/estate planning, family support for the caregiver.
Behavioral Health Support: We can also provide behavioral support for caregivers that are dealing with a loved one with a dementia diagnosis. Caregiving can become a full-time job and it is imperative that the physical and behavioral health of the caregiver is addressed in order for that person to continue care.
